Name	Description
Registration Form	Registration form with Name, Address/Postal Code, Email, Phone Number and DOB. Every team is required to fill out this form with their 2009/2010 roster.
Accident Claim Form	Did you get hurt? This form can help you with the process.
Waiver for Minors	Anyone under 18 that are playing in the WTHL must have this signed by a parent or guardian.
