Name	Description
Registration Form	Registration form with Name, Address/Postal Code, Email, Phone Number and DOB. Every team is required to fill out this form with their 2008/2009 roster.
Waiver for Minors	Anyone under 18 that are playing in the WTHL must have this signed by a parent or guardian.
